FMLA Advocate Needed (Resolved)

The other day, I recieved a phone call that my FMLA paperwork was completed. This paperwork is necessary it so I can modify my schedule to accommodate my wife’s need for continuous care and medical appointments. You can read about it here.

In this post, you can see that I did everything in my power to get a positive outcome and I thought I did.

However, my employer noted that section 11 and 12 were not complete.

I took the paperwork back in early in the day and brought more chocolate.

A medical professional came out and eventually told me that question 11 and 12 required time and research to fill out. He refused to give me a timeline.

This paperwork was filled out at the hospital for a continuous FMLA with little concern so I know it is not that difficult.

I am reaching out for an advocate that can help me get this completed.

    The form is pretty straightforward. I’d think your wife’s physician or therapist should be able to provide the information without much difficulty in general terms with caveats about the medical uncertainties surrounding her condition and response to treatment.

    Usually this is filled out by the medical professional and is no problem. Can you meet with the doctor, have a meeting?

      I got it squared away. The doctor avoided me by two layers. It was necessary to contact their management and a law firm.
